  • Status: Full-time, 40/hrs. week

What You'll Do:

As a Crisis Triage Coordinator, you will work closely with the Crisis Stabilization Services programming serving individuals in behavioral health crisis 24/7. Triage Coordinators may also participate as a member of the service delivery team to provide over the phone, in the unit, and/or field-based risk assessments, crisis interventions, safety planning, coordination of care including discharge planning, brief interventions, group work and potential placement for clients in Larimer County. Crisis Triage Coordinators will support our 24/7 Crisis Stabilization Unit and our crisis line answering calls from community members needing immediate crisis support/services, as well as providing real-time navigation of behavioral health services and resources to individuals. This position will be based out of our 24/7 Acute Care facility on the Longview campus and will be expected to cross train to cover multiple units within the facility, including Behavioral Health Urgent Care, Withdrawal Management, Crisis Stabilization, Mobile Crisis Response etc. as needed.

Duties include, but are not limited to:

  • Triage field response teams transitioning from the field to the facility for Crisis Stabilization Services provided at Longview Acute Care.
  • Answering crisis calls from individuals in the community needing immediate crisis stabilization services and other behavioral health services and resources.
  • Help community members navigate behavioral health services/resources in real time over the phone, connecting/transferring calls with other SummitStone programs and community partners.
  • Dispatch with our Mobile Crisis team to help conduct field-based crisis assessments and service placements as a dual-response team as needed.
  • Tracking our mobile crisis calls and helping to dispatch teams out into the community based on need and transition individuals to admissions to Crisis Stabilization Services.
  • Help to triage as part of our 24/7 crisis services team and provide support to other units within the facility as needed.
  • Participate in multi-disciplinary huddles/meetings.
  • Provide excellent customer services, adhering to SHP's values and philosophy of care.
  • Perform other duties as assigned.

Required:

  • Bachelor's degree in human services or equivalent experience.
  • A valid CO driver's license.
  • Knowledge of behavioral health disorders and services.
  • Ability to communicate with clients, co-workers, other agencies, and the community.
  • Ability to manage high stress crisis situations through de-escalation and/or physical management/safety training.
  • Ability to multi-task and carry out diverse job duties assigned each day.
  • Ability to understand situation awareness and potential threats.
  • Critical thinking and problem-solving ability.
  • Knowledge of diversity and inclusion with the relevant ability to engage, communicate, interact, and work effectively and cooperatively with people of different backgrounds, identities, and culturally diverse communities.

The typical hiring range for this position is $24.04 - $28.85/ hour, based on relevant years of experience.

Employees working at locations operating 24/7 earn a differential. Shift differentials are also available for evenings, nights, weekend and holidays.
