Job Summary

The post-transplant Nurse Practitioner is responsible for the care of post-kidney transplant patients. Patient care responsibilities include evaluation and management of patients who have received a kidney transplant, follow-up of post-transplant clinical testing and immunosuppressive management and ongoing discharge teaching and clinical assessment of this population. The NP will provide advanced practice care under the direction of transplant faculty. Close collaboration with the medical staff (including referring physicians) and in-patient clinical coordinators is essential to ensure appropriate follow-up care and transition of care within the clinic. When not in clinic, the NP will be responsible for managing patients' immunosuppression via lab review and patient discussions. The NP will be the key point person for patient and may refer the patient to social work or for insurance counseling as necessary. Patients will be monitored and documented through Apex/Epic systems.

Required Qualifications

Nurse Practitioner I

  • Minimum five (5) years outpatient experience caring for transplant patients.
  • Clinical skills (inc., hypertension management, incision/wound management, diabetes management: oral medications and insulin, diabetic teaching (diet, activity and blood glucose monitoring) phlebotomy, foley, JP drains.
  • Excellent communication skills (written & verbal) and professional conduct.
  • Computer literate (esp., MS Office: Word, Excel, PowerPoint, internet use) and willingness/ability to learn Apex computer systems, and other systems as necessary.
  • Excellent organizational skills, proficiency in meeting deadlines, ability to work independently and prioritize workload.
  • Excellent patient education skills.
  • Ability to multi-task and prioritize.
  • Flexibility to orient and work at all UCSF Medical Center locations.

Nurse Practitioner II
  • Minimum two (2) years NP experience, with at least one year previous NP experience in transplant, nephrology, or other relevant field.
  • Clinical skills (inc., hypertension management, incision/wound management, diabetes management: oral medications and insulin, diabetic teaching (diet, activity and blood glucose monitoring) phlebotomy, foley, JP drains.
  • Excellent communication skills (written & verbal) and professional conduct.
  • Computer literate (esp., MS Office: Word, Excel, PowerPoint, internet use) and willingness/ability to learn Apex computer systems, and other systems as necessary.
  • Excellent organizational skills, proficiency in meeting deadlines, ability to work independently and prioritize workload.
  • Excellent patient education skills.
  • Ability to multi-task and prioritize.
  • Flexibility to orient and work at all UCSF Medical Center locations.

Preferred Qualifications


  • Knowledge of kidney disease.
  • Knowledge of kidney transplantation.
  • Knowledge of immunosuppression.
  • Knowledge of infectious diseases in setting of immunosuppression.
  • Ability to safely manage above.
  • Ability to manage key disease processes related to ESRD/Transplant:
    • anemia,
    • rehabilitation for geriatric patients and post-op
    • cardiovascular conditions

License/Certification


  • California driver's license (CDL) if required for position.
  • Must hold current national certification from an accepted certifying agency and provide by first day of work.

  • Must hold a current and active RN and NP and Furnishing license in California.
  • Must have completed a BRN recognized graduate master's degree program as a nurse practitioner.
  • Must obtain an active and/or transferable DEA license to California and provide evidence by 90th day of clinical practice. (It is the responsibility of the employee to transfer their DEA license to their new place of employment if from out of state, via online DEA portal and provide evidence of transfer).
  • Must have submitted enrollment with CMS Medicare and Medicaid through credentialing by first clinical day. CCS enrollment as appropriate for pediatric environments.
  • Must hold current AHA BLS as minimum requirement.
  • State regulation requires that you have a supervising physician to practice.
  • Incumbents with less than 6 months of NP experience will be hired as a Nurse Practitioner 1.

UCSF Medical Center and Campus requires certain healthcare providers to be credentialed. In most cases this is done through its Office of Medical Affairs & Governance (OMAG) prior to the first working day. Credentialing and approval of the Standardized Procedures takes between 60-90 days but can vary.
