Job Summary: The Homeless Outreach Street Team (HOST) is a partnership between Integral Care, the Downtown Austin Community Court (DACC), ATCEMS Community Health Paramedics (CHPs), and APD engaging in outreach services with people experiencing homelessness in downtown Austin. HOST provides services in the community wherever individuals experiencing homelessness may be found, including city streets, parks, churches, at partner homeless service agencies, and at Pop-Up Resource Centers. HOST also plays an active role in the City of Austin's HEAL Initiative, which entails targeted outreach at homeless encampments identified for closure and assisting individuals there with their transition into bridge shelter.

QMHP responsibilities with HOST include assisting with emergency department, psychiatric, and criminal justice diversions; completing coordinated housing assessments; assisting with applications for housing, benefits, or vital documents; referrals to mental health or substance abuse services; assisting with scheduling appointments; providing transportation to appointments in HOST vehicles; connecting clients with shelter; assisting with basic needs such as food, water, and clothing; and providing coping skills training.

Ideal candidates will be adaptable, resourceful, and persistent. Experience working directly with people experiencing homelessness is preferred, but not required.

Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Education:

Required:

  • Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field.

License:

Required:

  • Current and valid driver's license and a clean driving record.

Hours: Full-time.

Workdays: Monday-Friday.

Workhours: 8:30AM - 4:30PM.

Salary: $22.35 - $23.47 an hour based on education.
