Registered Nurse Case Manager - Care Coordination - Edmonds

Swedish Medical Center is looking for a RN Case Manager (0.7 FTE, Day Shift) to work for the Care Coordination Department at Swedish Edmonds in Edmonds, WA.

$10,000 hiring bonus for eligible external hires that meet required qualifications and conditions for payment.

SCHEDULE: 56 hours worked per bi-weekly, operating hours 8a-4:30p PST. Week 1: 3 days worked; 8 hour shifts. Week 2: 4 days worked; 8 hour shifts. Rotating/every other weekend component to be worked (days vary based on department needs).

Provides clinical case management functions via a collaborative process that transitions patients to optimal clinical outcomes while assuring safe, cost-effective care. Demonstrates a daily commitment to the department's value of teamwork.

Required qualifications:

• Nursing Baccalaureate degree (BSN) from an accredited school of nursing required. Applicants without a BSN degree must actively pursue and attain a BSN within five years of hire. Continued employment by Swedish beyond five years is contingent upon attaining the BSN degree. Registered Nurses employed by Swedish prior to December 1, 2012 are encouraged to pursue the BSN degree, but obtaining the degree will not be a condition of employment.
• Three (3) years of Current relevant clinical experience.
• Washington Registered Nurse License upon hire
• National Provider BLS - American Heart Association upon hire

Preferred qualifications:

• Hospital-based case management experience.
