Advanced Heart Disease RN coordinator- CARDIOLOGY WA SHMC SPOKANE HEART INSTITUTE 2ND FLOOR at Providence Sacred Heart Medical Center & Children's Hospital Provides coordination/case management for heart failure patients across the continuum of care. Shares 24-hour responsibility for heart failure follow-up, including coordination of heart failure hospital admissions and monitoring clinical progress. Required qualifications:
