Job Summary

This position is located at the San Francisco Center for Jewish Living.

This position will work with the Care at Home Nurse Manager to ensure provider billing is accurate and follow-up care is submitted in a timely manner.

Post-discharge, will provide a 30-day longitudinal follow-up with patients and their families to ensure any post SNF d/c issues are resolved. A high degree of independence, motivation, professionalism, respect, and compassion are required to work with these high-risk patients in a complex medical environment. Must be able to manage and mitigate patient clinical and administrative issues and respond directly to the providers in an accurate and timely manner.

This role is part of a dynamic Care team within the Office of Population Health Care at Home in collaboration with the Division of Geriatrics. As an extension of the clinical team, the Health Care Navigator (HCN) is located at the San Francisco Center for Jewish Living (SFCJL). This position will assist UCSF providers with the management of UCSF patients who are admitted to SFCJL short-stay skilled nursing units.

The position will include daily coordination of patient updates, vitals, laboratory and radiology tests, coordination of internal and external records, care coordination upon admission, and discharge. In partnership with the SFCJL care management teams, assist with any barriers to patients receiving post-discharge care after UCSF hospital.

Coordinates with the provider team and primary care practice staff and assists patients in accessing transportation to medical visits, durable medical equipment, prescription medications, community resources etc.

Department Description

The Office of Population Health and Accountable Care (OPHAC) leads UCSF's development of innovative care models to improve the value of care we provide to populations and facilitate implementation across the health system. OPHAC is responsible for fulfilling the UCSF Health Accountable Care contracts and fostering other care partnerships.

The OPHAC Department is comprised of the following teams: 1) Clinical Programs (including Care Support, Care Transitions, Care at Home, Behavioral Health Collaborative Care, COVID Structure, and the Commercial/MA ACO Team); 2) Quality and Analytics; 3) Public Hospital Redesign and Incentives in Medi-Cal (PRIME) Program Management, and 4) Clinically Integrated Networks (CIN) and ACO Management.

Required Qualifications


  • BA/BS Degree including two years previous experience in a healthcare-related field; or an equivalent combination of education and experience (e.g. EMT, health educator, community health worker, social worker, health plan, medical corps, and clinical office staff)
  • Knowledge of medical terminology
  • Strong skills to comprehend and assess patients' grievances to quickly locate appropriate resources for assistance; in-depth knowledge of the organization and how to get issues resolved
  • Strong interpersonal and customer service skills; ability to communicate and resolve issues effectively with a diverse population of patients, staff, and physicians
  • Excellent analytical and problem-solving skills; ability to develop solutions and recommend changes and follow through with implementation
  • Excellent written and verbal communication skills
  • Skills to work independently in a fast-paced, demanding environment with minimal supervision
  • Ability to manage and oversee multiple tasks simultaneously, including high daily call volume
  • The flexibility to orient and work at all UCSF Health locations

Preferred Qualifications


  • Interest and/or experience working with older adults who may have cognitive or sensory difficulties
  • Prior experience developing workflow procedures and implementing systems to improve operations
  • Proficiency with EPIC APeX applications
  • Proficiency with MyUnity EMR
  • Strong knowledge of Patient Rights & Responsibilities, Joint Commission standards, and Centers for Medicare/Medicaid regulations; knowledge of medical Terminology; strong knowledge of data collection, compilation, and analytical techniques
