About the Role:

The Managerof Care Management will be responsible for all Care Management programs' supervision and oversight for all care management programming. This includes ensuring compliance with regulatory standards for Case Management and Disease Management programs. This person will work closely with operations, Clinical Services, Care Management delegation, and compliance, ensuring all CM services are provided to all referrals.

Under the supervision of the Director of Care Management, the Manager of Care Management is responsible for supporting all daily operations of the Care Team(s). The Manager provides leadership and support to maintain the care team's focus on reaching established market/LOB goals.

Responsibilities:



  • Leads and supervises Care Management Operations to ensure care management activities are compliant with relevant regulations and requirements and consistently deliver high quality care to members.
  • Oversees compliance with internal and client CM Audits
  • In collaboration with the Clinical/Quality operations leads and care management staff, will identify and address opportunities to streamline workflows that result in accurate, high quality production standards and improved results in cost savings, or outcomes measurement.
  • Develops effective working relationships with and provides high quality service for members
  • Works with clinical services leadership to create process flows supporting CM program care coordination protocols, ensuring timely completion of all inquiries.
  • Attends and participates in Clinical and Quality department meetings as assigned
  • Tracks and monitors all metrics related to CM programing, implementing plans for improvement as necessary.
  • Works in collaboration with Director to oversee new employee hiring orientation and competency evaluation
  • Serves as a Market resource and manages issues/problems escalated by staff
  • Holds regularly scheduled Care Management staff meetings



Requirements for the Role:



  • 3 years experiences as a Manager/Supervisor of Care Management
  • Minimum 3 years experience in a managed care environment across multiple lines of business (Medicare Advantage, Managed Medicaid, Dual SNP, Commercial, etc.), with at least 2 years Dual Special Need Plan (DSNP) experience
  • Active and current unencumbered license as Registered Professional Nurse in state of practice
  • CCM certification
  • Working knowledge of regulatory, Medicare, Medicaid and third-party payer compliance requirements
  • Excellent leadership skills and experience required
  • Ability to establish priorities, meet deadlines and maintain productivity
  • Ability to form positive collaborative relationships with Market and Care Team colleagues
  • Ability to problem solve in a proactive creative manner using sound judgment based on factual information and clinical knowledge.
  • Ability to work independently and within a team structure
  • Excellent interpersonal skills and communication style
