About the Role:

The Director of Care Management is responsible for developing systems and processes for Care Management clinical operations, as well as managing the activities related to cost of care, performance management and clinical quality improvement. This person must be committed to improvement and promoting the health of the population served.

The Director evaluates and ensures that clinical and non-clinical resources are used appropriately and effectively, as well as overseeing the collection, analysis and reporting and quality data related to care management, utilization management, quality improvement and performance improvement. Additionally, this position promotes interdisciplinary collaboration, fosters teamwork and champions service excellence.

Responsibilities:



  • Accountable for clinical operations and program outcomes of the DSNP Care Management department.
  • Accountable for ensuring compliance with contractual, accreditation and regulatory requirements
  • Define and execute strategies to Improve Population Health outcomes
  • Mentors and develops skills of care management team members.
  • Responsible for the provision of quality services by maintaining appropriate resources, staffing levels, competency of staff, maintenance and performance of equipment, and training and education of staff
  • Promotes interdisciplinary collaboration, fosters teamwork and champions service excellence
  • Provides direction and oversight to all team members including, Case Managers and Community Health Partners
  • Responsible for approving and managing the day to day local level operational budget.
  • Responsible for overseeing the education of physicians, managers, staff, and members related to the care management process at the local level.
  • Participates in this evolutionary process by constantly identifying future needs of customers.
  • Uses data to drive decisions, plan, and implement performance improvement strategies for care management
  • Identifies improvement opportunities and facilitates process standardization to promote outcomes in alignment with the strategic and operational goals of Cityblock Health
  • Establishes and standardizes defined policies, procedures and processes to optimize efficiency and maximize productivity
  • Directs and participates in orientation training for new employees and re-training as needed
  • Establishes and maintains working relationships with health plan partners



Requirements for the Role:



  • Current unencumbered RN license In State of operation
  • Active CCM certification
  • 5+ years Care Management leadership experience
  • 7+ years of Managed Care Experience, with at least 3 years experience with Dual Special Needs Plan (DSNP)
  • Experience managing to NCQA policies and standards
  • In-depth knowledge of Medicare Advantage, Managed Medicaid, and Commercial LOBs.
  • Progressive experience in management, with demonstrated ability of supervision of multi-skilled personnel in a fast-paced environment both onsite and telecommute staff
